I created the Google Form and sample introduction letter below when we filed our Firewise application for collecting Individual Home Wildfire Assessment data. It may be useful to others forming new FWCs:
The form can be completed online or printed and handed out to residents for manual completion and return.
If you haven’t had any experience with Google Forms, I may be able to help you get started. The form responses are collected in a spreadsheet which must then be summarized for use in the Firewise Application.
